Services
The Perry Township Trustee’s Office is here to help the community’s residents from all walks of life who find themselves in need of temporary help. Services are provided through purchase orders (vouchers) for shelter (rent), utilities (gas, electric, water), food, medical, and burial. Call for assistance and to make an appointment to see if you are qualified.
Documents Required for Assistance Applications
All items listed are needed every six months for renewal. All adults in the household must be present during the application interview and all documents must be presented for every individual in home.
– ID and Social Security Card, original, not copies
– Lease or mortgage (Lease must state who is in the home/or statement from landlord required). If no lease, must have written statement from property owner stating living arrangements, who lives there, how much rent is paid, what it includes, signed, dated and including property owner’s address and phone number.
– If on subsidized housing such as section 8 documents with how much is paid by tenant and how much section 8 pays. Additionally, utility information required.
– Proof of income from the past 30 days, see below for more information
– Pell Grants, Student Loans – Subsidized or Unsubsidized
– Child Support – Proof from child support office even if not receiving
– Disability – Requires current award letter or current bank statement if direct deposit
Applicant must account for outgo of all income for the past 30 days of income for everyone. Bring bank statement, money order, receipts, bills, etc. Proof of food stamps case number with list of all persons receiving or not receiving and the amount the household receives or proof you have applied for everyone in the household.
Your application can only be completed if all information and documentation is recieved. Everything MUST be in paper or printed form at the appointment.

– Work income, including tips
– TANF (formerly AFDC)
– Odd jobs, even if paid through cash
– Sick pay
– Relatives and friends contributing to help
– Church assistance
– Workman’s compensation
– Social Security
– Unemployment
– Child support
– Vacation pay
– Tax refund
– Pension
– Bartered goods
– Other social service agencies
– Payments made on bills
– Plasma donations
– Stimulus
– Any other income made for the household
Statements of assistance from friends or family must be in writing with name, address, phone number, and date.
